Beloved son of Carrie Cook. Loving father of Jason Cook. Fond brother of Debra, Steve, Scott, Kelly, Pam, and David. Further survived by nieces, nephews, other relatives and friends.
Graveside services will be held at Mt. Olivet Cemetery, 3801 W. Morgan Ave. on Friday, August 23, 2019 at 1:30 PM. Please meet at cemetery office.
