Loving brother of Connie Smith. Also survived by nieces, nephews, other relatives and friends. Predeceased by 4 siblings.

Visitation at the funeral home on Monday, December 13, 2021 from 11 AM - 2 PM. Interment Highland Memorial Park, New Berlin, WI.

Jack was a proud Teamster 200 and an avid sportsman who loved to hunt and explore the outdoors.

The family would like to give special thanks to the staff at Ascension WI Greenfield Hospital for their compassionate care of Jack.
